This is to announce that applications for new 10 year Polish Passports, accompanied with all required documents, may be submitted to the Consular Division of The Embassy of The Republic of Poland only after an individual appointment has been agreed upon by phone.

The appointments  should  be  made on  the  telephone ,

Number:  (  03 )  72-53-120,   
Monday, between  10.30 – 17.30,  and
Wednesday, between  9.00  and  15.30

Applications for Polish Passports should be properly completed in  the polish language,  by applicants themselves,  according to a specimen placed on the notice board of the Consular Division of  the  Embassy.  Consular officers will not assist the applicants in completing  passport applications.
