PLEASE NOTE: It is no longer necessary to make an appointment for a passport application  as of  Thuesday, 26 January 2010.
 

ALL APPLICATION FORMS FOR PASSPORTS MUST BE DULY COMPLETED IN POLISH AND SIGNED BY THE APPLICANT AND ALL NECESSARY DOCUMENTS PREPARED.
